The Centers
Job Summary
Provides primary medical care to adults of all ages (and children, if family medicine) which includes chronic disease management, preventative care, and care of acute medical issues. The Centers is a Federally Qualified Health Center (FQHC) with a focus on providing comprehensive care for medically underserved individuals in our community, as well as a Community Behavioral Health Center (CBHC). Our clinic serves many individuals with mental health conditions and substance use disorders. The Primary Care APP will work as part of an integrated team with our psychiatry providers, counselors, case managers, addiction medicine team, harm reduction team, dental team and health team (nursing, medical assistants, clinical pharmacists) to provide comprehensive care for our patients.
There are opportunities to practice addiction medicine, as well as infectious disease (STI screening and treatment, HIV PrEP and treatment, Hepatitis C treatment) in the primary care setting. In addition, there are opportunities to provide sexual and reproductive health services (LARCs, colposcopy) and perform common primary care procedures.
This role qualifies for HRSA National Health Service Corps (NHSC) scholarship and loan repayment programs.
Essential Job Duties & Responsibilities
- Complies with the Center’s organization-wide and Department of Primary Care’s standards of care and protocols
- Utilizes evidence-based primary care and nursing practice or physician assistant practice appropriate to the setting.
- Identifies professional learning curve and routinely engages in formal and/or informal clinical supervision appropriate to needs.
- Completes clinical work tasks during work hours each day.
- Provides assistance and direction to agency staff in establishing treatment/service plans, as needed. Knows and utilizes system resources. Identifies system limitations and problem solves to meet client needs.
- Responsible for accurate, timely, and complete documentation in-patient records in accordance with organization’s policy.
- Participates in shared call system with other providers to provide continuous coverage for patients on a 24-hour basis. Call includes phone availability after hours and is rotates among the primary care providers.
- Maintains professional composure under stress.
- Manages conflict at all levels.
- Manages conflicting demands.
- Provides patients with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verifies and documents that the patient understands diagnosis, treatment, and follow up recommendations.
- Renders care to patients in accordance with evidence-based quality standards, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.
Job Qualifications
Nurse Practitioner Qualifications:
- Must have a Master's or Doctoral degree in Nursing and valid Ohio license
- Must have a current, full, active, and unrestricted license, or otherwise recognized, as an Adult Nurse Practitioner or Family Nurse Practitioner
- Must have current, full, active certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. [The certification must be in the specialty to which the individual is being appointed or selected.]
- Must be able to obtain a valid Drug Enforcement Agency (DEA) license
- Must work within a Collaborative Care Agreement with a supervising physician
- Primary Care Experience is strongly preferred
- May be required to travel between sites as necessary
Physician Assistant Qualifications:
- Graduate of a PA training program which is accredited by the ARC-PA or one of its predecessor organizations.
- Possess a minimum of a master's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the US Department of Education.
- Current, full, active certification as a Physician Assistant by the National Commission on Certification of Physician's Assistants (PANCE) - OR - a recent PA graduate who is eligible to take the PANCE
- Must be able to obtain a valid Drug Enforcement Agency (DEA) license
- Must work within a Physician Assistant Supervision Agreement/Collaborative Practice Agreement
- Primary Care Experience is strongly preferred
- May be required to travel between sites as necessary.
