Reintegration case management uses a treatment focused approach that collaborates with criminal justice systems to stabilize mental health symptoms and reduce recidivism. The Case Manager uses a client-centered approach that provides intensive services while the client is still incarcerated and continues when the client is released back into the community.

In this role, you’ll
• coordinate with other professionals including parole officers, probation officers, court staff, and forensic liaisons
• refer and link clients to the Agency and other community resources to address basic needs such as food, clothing, housing, and benefits
• coordinate treatment planning to identify goals
• utilize evidence-based strategies to support mental health recovery
• remove barriers that prevent access to care
• monitor and facilitation of addressing need across the healthcare spectrum to include physical health, mental health, and other specialty healthcare needs
• work with clients with high demand

Qualifications

• High school diploma (or equivalent) and three years of direct case management experience, or in lieu a bachelor’s degree in a related field
• Good interpersonal, written and verbal communication skills
• Understand mental illness, symptoms and impact on behaviors; understand concept of recovery/resilience
• Have an ability to think critically in a fast-paced environment and manage time effectively.
• Be comfortable transporting clients (valid driver license, vehicle, and insurance a must)
