To facilitate Reserve Care's (RC) efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in end-of-life care for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education, collaboration, consultation and possible research related to end-of-life care.
WHAT YOU WILL DO:
- Participates in analyzing and evaluating clinical practice standards in terms of cost and quality
- Provides independent direct nursing care to patients, within the Nurse Practitioner role as delineated in State of Ohio
- Participates in development and implementation of new standards of practice
- Maintains and promptly completes all required records and reports
- Participates in Transdisciplinary team
- Serves as a consultant to staff caring for patients in long-term care settings and home settings
- Assists in identifying learning needs of staff through formal and informal needs assessments
- Maintains a thorough knowledge of the current practice and literature relating to end-of life care and facilitates the sharing of that knowledge with staff
- Maintains a standard care agreement with RC Medical Director
- Participates and/or chairs RC committees as needed
- Participates in quality improvement activities
- Maintains and cultivates professional relationships in healthcare that strengthen the agency's strategic marketing plan
- Fulfills professional growth through involvement in professional organizations, presentations and publications
- Participates in agency research endeavors related to patient care
QUALIFICATIONS, EDUCATION, EXPERTISE AND KNOWLEDGE
- Maintains a current Nurse Practitioner licensure in state of Ohio
- Maintains a current Certificate of Authority in state of Ohio
- Master of Nursing Degree
- Minimum of two years in clinical setting
- Ability to demonstrate excellence in written and oral communication
- Ability to establish and maintain collaborative working relationships with RC employees, community and healthcare providers
- Ability to provide own transportation
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to RC and/or RC's insurer

CONDITIONS OF EMPLOYMENT:
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver’s license
- Ability to provide proof of eligibility to work in the United States.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics working with patients. This work requires stooping, reaching, standing, walking, lifting, fingering, grasping, and feeling; vocal communication; hearing; visual acuity and color perception. Employee understands and accepts possibility of exposure to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patients and family situations.
